Sacrament Records Request
"Preserving Your Spiritual Milestones."
Sacraments mark significant steps in our journey of faith, and we know that having a record of these moments is important. Our Parish keeps records of all sacraments celebrated here, including Baptism, Confirmation, First Holy Communion, Matrimony, and Anointing of the Sick. These records are often needed for religious, legal, or personal purposes, such as enrolling in Catholic schools, preparing for marriage, or even tracing family heritage.
If you need a copy of a sacrament record, we are here to help you with the process.
How to Request a Sacrament Record
To request a sacrament certificate or record, please follow these steps:
-
Complete the Online Sacrament Records Request Form: Please fill in the details of the sacrament (e.g., Baptism, Confirmation) and the date or approximate year it was celebrated.
-
Provide Identification: To protect the privacy of our parishioners, we may require you to show us a valid identification of the person making the request. If you’re requesting a record for a minor or deceased family member, please provide proof of relationship (such as a birth certificate or death certificate).
-
Processing Time: Please allow up to two weeks for us to process your request. If you need the record urgently, contact our Parish office, and we’ll do our best to accommodate.
Frequently Requested Sacrament Records
-
Baptism Certificate: Often needed for other sacraments, school enrollment, and religious education.
-
Confirmation Certificate: Typically required for marriage preparation and certain Church-related roles, such as being a godparent or sponsor.
-
Marriage Certificate: May be requested for both religious and civil purposes, including marriage counseling and immigration requirements.
-
What if one parent isn’t Catholic? We warmly welcome families from all backgrounds. At least one parent should be committed to raising the child in the Catholic faith.
-
How many godparents can we choose? The Church requires at least one Godparent but it is common to have two – one male and one female. Godparents must be practicing Catholics, confirmed and at least 16 years old. Additional friends or family members who are baptized Christians can act as honorary godparents or "witnesses".
-
Is there a cost for Baptism? There is no fee for receiving Baptism. It is normal for the priest to be offered a donation (“stipend”) and our Parish asks for a contribution towards expenses. However, no-one will be prevented from receiving Baptism for financial reasons.
